How to Build a Clinical AI Chatbot as a First-Time Founder

A step-by-step playbook for founders turning a clinical protocol into a safe, rule-based AI chatbot — from sensor inputs to human-in-the-loop escalation.

Most first-time founders building health AI start in the wrong place: they pick a model before they pick a protocol. The 2026 diabetic-foot remission paper is a useful counter-example, so let's translate it into a how-to you can actually follow when you're moving from a research idea to a working chatbot.

Step 1 — Anchor on a clinical protocol, not a model

Pick a published guideline or remission protocol and write it out as a flowchart on paper. If you cannot describe the decision tree without using the word "AI," you don't have a product yet — you have a demo.

Step 2 — Define the daily loop

Borrow the four-move loop: capture, classify, recommend, escalate. Each move should be one function in your codebase with a clear input and output. Resist the urge to merge them into one big prompt.

Step 3 — Pick three signals, not thirty

The remission paper uses step count, skin temperature, and shoe-wear time. That's it. For your domain, choose the smallest set of signals a clinician would actually act on, and write down the exact thresholds (e.g. a 2.2°C left-right delta).

Step 4 — Make the brain transparent

Use rule-based logic for the safety-critical calls and reserve LLMs for natural-language framing. Reviewers, regulators, and clinicians can all read an `if/else`. They cannot read a transformer.

Step 5 — Fail safe, not silent

Missing data should trigger a reminder to the patient and a flag to the clinician — never a guess. Write the failure modes before you write the happy path.

Step 6 — Put a human on the call

Every escalation routes to a named, accountable clinician. This is the part that turns a chatbot into a medical workflow.

Step 7 — Run a feasibility pilot before a trial

Ten patients, four weeks, one clinic. Measure adherence, false-alarm rate, and time-to-escalation. Use that data to design the real study.
